Advanced technology allows us to monitor your dialysis sessions remotely, ensuring that each session is conducted safely and effectively, and allowing for immediate intervention if needed.

Experience Personalized Home Visits and a Tailored Dialysis Schedule

You may be receiving dialysis at home, but you are never alone. Questions concerning your equipment or health plan can be answered by your care team. Your technical support team, home dialysis nurse, and other members of your care team will work to provide answers and solutions to your health concerns.

Nephrologist

Your nephrologist is the head of your care team. They are responsible for prescribing your dialysis treatment, medication, correct dialysis training and equipment. You’ll have monthly telemed or live clinical visits with your nephrologist to ensure careful monitoring of your condition, assess your treatment plan and make any necessary changes to your care plan.

Home Hemodialysis (HHD) Nurse

Your HHD nurse will be an integral part of your home dialysis training and care plan. Each patient is assigned an HHD nurse. They will be your primary point of contact and follow up once at home. Your nurse will focus on the following: HD training, dialysis theory, medication administration, blood draws, blood testing and blood shipping, access care, dialysis documentation and troubleshooting.

Social worker

Your social worker is a specialist in helping patients adjust to all the latest changes — financial, physical, and emotional — that comes from starting home dialysis. A social worker can offer additional information, support, and aid in the successful integration to home hemodialysis. They will also be able to assist you with understanding insurance, working while on dialysis, and provide resources for medication, travel arrangements, and coordinate transplant referrals.

Dietitian

Your renal dietitian will collaborate with you to create a customized, kidney-friendly diet that provides the nutrients you need—and incorporates some of the dishes you love! They will provide supplements and advise you on how to improve lab results where diet plays a role.

Biomed

A biomed will be responsible for maintaining and servicing your dialysis equipment. When the need for service arises, a biomed technician will be available to assist you over the phone. If further diagnosis or intervention is required, a technician will be sent out to your house to repair and maintain your equipment.
